Nu Horizons of Southern MN provides Adult Corporate Foster Care serving up to four individuals per residence.  We offer single and double furnished rooms, but are willing to remove our items in the event the consumers prefer their own. 

 We encourage families to come in and help personalize the individual rooms, so they have the continued comfort of home and family like atmosphere.  We believe that a healthy and caring atmosphere is the foundation of daily living that can enhance responsible decision making and encourage independance.

Our staffing ratio is no less than 1-4 with a minimum of 2 hours a week of one on one time per individual.  Additional one on one time is available as needed.

                                   A 24 hour on-call LPN and RN is available for all of our homes.

Nu Horizons of Southern MN implement house rules and schedules that provide expectations for continued involvement within the home and community. These rules also encourage respect and safety measures for daily living.

Group Meetings or house meetings are held once a week within the homes.  These meetings are meant to be a positive time for the consumers to voice their opinions on upcoming activities and events and to inform staff and roommates of any concerns one might have. 

Our goals with these meetings are to recognize and commend the progress that the consumers have set for themselves as individuals as well as the goals for the group as a whole.

Nu Horizons of Southern MN accepts the following, but not limited to: Private pay, CADI Waiver, CAC Waiver, Elderly waiver, and MR/RC waivers.

We are currently full at our 406 NW Park Ave-Elysian home.This location is focused on moderate to highly independant residents.  Some of the residents transition from this home back into the community on their own.  We are single staffed for 24 hours a day.   

We are currently full  at our 406 1/2 NW Park Ave-Elysian home.  This location is focused on residents with behaviors, at risk for elopement, etc.  This home is double staffed for 12 hours a day(daytime peak hours).

We are currently full at our 19082 State Hwy 60-Waterville home.  This location is focused on residents that are mild to highly independent.  Most residents at this home have up to 4 hours of alone time, however mild behaviors,  (Alone time is based on individual consumer needs independance level).  This home is double staffed for 12 hours a day (daytime peak hours)

We currenlty have one opening at our 511 Hwy 60 West-Elysian home.  This location is focused on mild to high medical needs and behaviors.  This home offers private suites that include a private bath.  Our site is full handicap accessible with ramps, wide doorways, hallways, and roll in shower. This site is double staffed for 12 hours a day.
